Seribu North Jakarta Super Puma PK-PUH Pelita Air Services 4 0 0 0 I CRS T Approximately 10 minutes after take off the pilot saw the MGB temperature indicator slowly increased to the maximum followed by MGB TEM light "ON" on the caution warning panel, and a few seconds later the MGB pressure dropped to zero. Then pilot decided to make emergency landing.

Having realized his error the pilot stopped the a/c and requested to pull m/c to tower. Further examination found F.O.D (Brass) in engine no.1.

Increasing altitude caused the engine parameter # 2 to go to max. (red light iluminated on N1 and EGT). The crew take action follow by an emergency landing.

It was reported that part of the taxiway lightings were off. Near the end of the taxiway the a/c stopped with the nosewheel and left main gear on the R/W shoulder.
18 11-Jun 1999 Wamena , Irian Jaya KAMOV RA-31029 Manunggal/Trigana 3 3 5 1 A CRS H/T On a flight from Wamena to Mulia, 5 minutes after take off the ATC lost contact with the a/c. Two hours later the a/c was reported missing and ATC started searching. The a/c was found crashed inland of Irian Jaya.

One minute later a second bump sound was heard and it was observed that engine number #3 stalled, indicated by eratically fluctuating indicator.

26 27-Sep 1999 A. Yani, Semarang Cessna PK-DCK Deraya 1 3 0 0 A LDG H During landing roll the a/c experienced hydroplaning and was uncontrollable and went out of the R/W. All gears collapsed and the propellers hit the ground, damaging the propellers and also the lower surface of the a/c.
